Are you living in Lithuania already? If so you should read the following explanation, it should guide you through your first steps of applying for a work permit. http://livingin.lt/en/work/artic... There is a section here called documents required for a work permit.
If you want to call the relevant institutions, they are
1. Ministry of Social Security and Labour, http://www.socmin.lt/
